Job Description

Care Coordinator (Domiciliary Care)

Reading, Permanent

Salary £20,100 - £24,500 per annum

Job Summary: Our client is currently looking for a Care Coordinator to join their busy team to responsibly and appropriately manage the coordination of the care delivery team. The right candidate will ensure provision of commission services to the people they support and be the first point of contact for day to day operation requirements of care.

Key Responsibilities

  • Ensure the care team have appropriate shifts allocated and any gaps in schedules are filled with an appropriate carer, stepping in when needed
  • Monitor and maintain the electronic call monitoring systems with incoming and outgoing times
  • Rota management
  • Coordination, collection and distribution of charts
  • Staff supervisions and appraisals
  • Assist in the on-call service on a rota basis
  • Monitor compliance stats of care delivery team

Ideal Applicant

  • You will be experience in the following roles; Care Coordinator, Care Team Leader or Senior Care Assistant
  • Preferably have NVQ level 1, 2 or 3
  • Able to provide a high level of domiciliary care
  • Caring, understanding, and able to grasp the importance of your role to your customers
  • Good interpersonal and communication skills
  • Self-motivated
  • Organised
  • Must hold a valid drivers license
  • Comfortable with being on-call and covering service visits to users
