Care Coordinator

Job Description

We are looking for a Care Coordinator for a charity based in Trafford. The charity is dedicated to supporting people and children. They support over 200 people each year. They have residential services, community services, home care and clubs that run on weekends and evenings.

You will be working as a Care Coordinator which will lead shifts and coordinate shifts. You will be completing assessments and understanding the needs of the people the charity supports. You will lead the care planning and work with health and social care professionals and families. You will also be involved in creating and developing behaviour management support plans. You

You will need to have completed your Level 3 CQF/NVQ in Health and Social Care to be considered for this role. You will need to have had experience in working at a Senior Support Level within a health care setting. You will need to be able to demonstrate previous experience of leading a team.

This role is a full time role 37.5 hours - This will be working on a shift system. There will also be some senior on-call duties with there is additional payment for.

The salary for this role is £22,000.

You will have up to 28 days holiday per year, enhanced pay for bank holiday, and mileage allowance.
