Job Description

BS Social Care are looking for two Care Team Assessors to work with a charity organisation within the Cambridgeshire area to assist the NHS hospitals with safe and caring discharge submissions. This role allows you to enable service users to go back home as soon as they can and receive care from where they enjoy being the most.

About the employer; this employer is a charity organisation supporting Carers who are supporting their loved ones who are people with any condition at all ages from children to older adults. This organisation looks after both the service user and their family and friends.

About the job; you will be a very important person who is able help people shorten their stay in hospital where safe to do so. You will work with the professionals of the NHS healthcare service and assess your service user to bring them home. You will work with both the hospital teams and your own teams to ensure all procedures are followed using the Ofsted and CQC standards to carry out a discharge and then you will either create a team for this service user or care for them yourself depending on the needs.

About the ideal candidate; ideally you will hold a NVQ 2 or 3 at the least, you must be a driver for the nature of the role and ideally you will have either similar experience to this role or you have worked in a hospital or you have worked within a care setting. You will be flexible, you will be team player and you will be confident enough to liaise with all levels of professions.

In return, this organisation will offer a permanent and full time contract paying £17,000 - £21,500 depending on experience and qualifications. They will also offer 2.5 weeks of a full in depth induction alongside further training and refresher training and personal development opportunities. They also offer a fuel card, a charity environment, annual reviews, great quality training and 28 days annual leave including bank holidays.

This role will require you to work 8am - 4pm / 9am - 6pm / 12pm - 8pm and alternate weekends. We are open to people applying for part time hours but you would need to adhere to the shifts mentioned just in a part time capacity.

We are also recruiting for a Care Booking Advisor for the same organisation paying £19,500 - £25.500. This role is to oversee the coordination of service user daily calls for the service users being cared for via this organisation.
