Do you have experience in the social care / care industry within a Case Co-Ordinating role? Do you hold a NVQ Level 2 or above in Health and Social Care or equivalent?
Working at one of the client's sites in the Warrington area, you will be expected to support, develop and coach service users to both achieve and maintain sustainable outcomes. In addition to this, you will also promote strong engagement with service users alongside both developing and strengthening with partner agencies.
Benefits for this role include:
A competitive basic salary ranging between £17,600-£20,000 per annum (dependant on experience).
Equivalent to 5.6 weeks holidays allowance (although there is a need to work bank holidays and a strict 4 on, 4 off shift pattern basis).
Day to day duties in this role include:
Assist in the provision of high quality housing management functions.
Carry out high quality service user reviews facilitating the planning, implementation and monitoring of their progress through and beyond their journey within the company.
Ensure effective networking relationships are established to expand the range of services available, attending and participating in case reviews, meetings, and statutory conferences as required.
Ensure any issues with the building and site - whether that be the reporting of repairs, requests from service users and all complaints are addressed.
In order to apply for this role, you MUST have the following skills and experience:
Hold a NVQ Level 2 or above in Health and Social Care or equivalent
Awareness of Health & Safety issues
Knowledge of working with young people or a vulnerable client group alongside any experience of issues that affect young people.
It would also be desirable for the right candidate to hold an enhanced DBS check as this is part of any successful applicant's application.
