Job Title: Contract Manager

Band: Band 8a

Responsible to: Head of CHC Business

Accountable to: Head of CHC Business (support provided by Associate Director of Contracts)

Responsible for: To be responsible for the negotiation, development and monitoring of a range of healthcare contracts.

Location: Acton House. Requirement to work in other offices across Herefordshire and Worcestershire STP

1. Key Objectives

" Highly motivated and enthusiastic individual to lead on Individual Patient Activity (IPA)/ Continuing Healthcare (CHC) Contracts and Brokerage Team functions. The post holder will have relevant knowledge and experience in Contracting and Brokerage/Commissioning working within the CHC Team
" The role involves the relationship management of IPA providers of care home services; other residential settings and homecare and will be liaising with the CHC/Complex Care clinical team, CCGs and providers as appropriate.
" To be responsible for contract negotiation and management, skilled in building relationships with strong interpersonal skills to deal with highly complex and sensitive matters that arise in day-to-day business.
" Lead on all aspects of the commissioning cycle and be able to critically appraise service delivery to develop high innovative local solutions. Demonstrate significant experience of successfully commissioning high quality services and, given the specialised nature of the portfolio area, expert knowledge of Health and Adult Social Care, clinical guidance and service delivery.

" To work in partnership with other colleagues across the Council and CCG, Providers, customers and other stakeholders to develop and re-design services that ensure defined patient outcomes are achieved.

" Analysis of data, a good understanding of healthcare pathways, querying variances from plans or targets, managing performance issues and pursuing actions to recover performance, authorising payment of invoices.

" Manage contracts directly with providers including negotiation, development, and monitoring of a range of Continuing Healthcare projects contracts held by Hereford & Worcestershire Clinical Commissioning Group (CCG). Within these management arrangements, the post-holder will contribute towards the CCGs' commissioning strategy by supporting a programme of activities designed to deliver recurrent efficiencies and productivity gains within a cultural context of continuous improvement and improved patient experience.

" Lead the monitoring of and subsequent contract variations arising from the implementation of contracts, grants, allocations and KPI's


" Plan, negotiate and agree the financial value of contracts with appropriate providers on behalf of the CCG, keeping within the budgetary allocation for the service portfolio.

" Analyse and interpret complex data such as contract activity, demand and capacity data, finance or performance data, present findings and producing reports order to query variances or anomalies, inform planning decisions and manage provider performance;

" Maintain an overview of all allocated contract performance, in order to report to the CCGs' Performance bodies and CCG Boards and to inform planning and commissioning decisions;

" Ensure the CCG discharges its responsibilities under the relevant CCG agreement/constitution for contracts within the post holder's portfolio to which the CCG is party to a Collaborative Commissioning agreement. This will include agreeing activity plans and contract values, monitoring performance and expenditure against commissioning budgets and raising performance issues;

" Contribute to service reviews, service specification development and pathway redesign to help ensure that the best value for money and highest quality health services are secured within the available
