My client is quickly expanding and looking for a Care Coordinator to join their team - they primarily provide support for vulnerable client groups within their own homes and specialise in elderly, dementia, learning disabilities and physical disabilities.

ABOUT THE ROLE

Salary: Up to £24,000 per annum
Location: Hemel Hempstead
Contract: Permanent, full time Mon-Fri 9am-5pm (plus on call duties)


ABOUT YOU

Must have experience within a care coordinator position
Background within rostering and using packages such as CM2000 or CarePlanner (or something similar)
Due to the vulnerability of the client group you must be double vaccinated and able to clear an enhanced DBS check
