We are currently recruiting for two experienced and dedicated Home Care Coordinators to lead a team in delivering high quality, person centred, Domiciliary and Outreach Services.
Working for a fantastic organisation with over 16 years` experience in their field. They specialise in domiciliary care, providing professional and person-centred services and offer a wide range of high quality, person centred and outcomes focused Support services to support people with Autism, Asperger`s, learning disabilities and those with profound and complex needs.
As the Care coordinator your main base will be the Office, based in Bolton you and your team will provide the best professional and person-centred care. Your other duties will include management of electronic rota systems and people planner, ensuring weekly rotas are sent out to staff in a timely manner, following up the initial contact between the care worker and client to evaluate the Care Plan and ongoing needs, to develop and maintain relationships with your clients and care staff team. You will establish this by undertaking regular telephone assessments, monitoring the service user`s satisfaction and care worker performance and development. Your duties will also include on call duties shared within the team.
The successful candidate will have relevant leadership and electronic rota management experience, you will hold a Level 3 in Health and Social Care and hold a full UK Driving Licence.
In return to your hard work you will work Monday - Friday 9:00am-5:00pm, receive a salary of between £18,500-£20,500 plus on call allowances, further training and development!
