Job Description

BS Social Care are recruiting alongside a renowned national health and social care provider for a new and unique role in the Teesside area.

As a hospital liaison worker, you will be based within James Cook hospital to assess those who present needing mental health intervention and/or treatment, including wraparound services.

You will be a key part in the hospital team's delivery of mental health assessment and treatment delivery ensuring that the patients and service users access the agencies that they need in the community. This will both ensure that the service user has the best chance of working with the services available which will help with their emotional wellbeing, and on a strategic level should help prevent possible hospital admissions in the future.

This is a role that, more than anything else, requires an understanding of multi-agency working and coordination. An understanding of other service providers is key in this position and you will communicate with agencies on a daily basis.

Although signposting is vital, this is only one part of delivering this service. There will be some follow on with service users, working with them with to ensure they access the services they need after you have referred them.

You will have experience in working in and around mental health services and a knowledge of signposting and enabling service users to empower themselves. You will need to have knowledge around crisis intervention and an understanding of the mental health act. Driving will be a key part of the role so a full UK licence is necessary.

While this is initially a fixed term contract position for 12 months, we are hoping this is going to be a role that is going to be around for longer within James Cook hospital. It is a fantastic opportunity to work alongside all workers and agencies in the hospital setting and also community services. This really is a unique role in an expanding mental health service delivery around Teesside.
