Overview of the role-

Our client is opening a supported living project in, Leeds, this is a 14 bed service offering intensive support to individuals with complex mental health issues.

We are looking for flexible staff to join the team in providing outcome focused support, enabling people to live as independently as possible.

Various hours and shifts available, including full time and part time contract as well as Zero hour contracts.

We are looking for flexible staff, the hours available include both days and nights, worked over a 7 day rota,

We are looking for committed and person centred individuals to enable the people we support to be as independent as possible and maximise opportunities.

Full induction training will be provided however some experience of working in a similar setting is desirable as the circumstances of the people being supported will be complex.

All appointments are subject to an enhanced DBS check and satisfactory references.
Induction training will take place for a week on 9th November 2020

Roles and responsibilities-
To provide practical empowering support to clients in an enabling way, including encouraging and supporting them to undertake the day to day aspects of daily life, including when required, helping them to take medication in line with the support plan and standard operating procedures

To work in line with the agreed support plan for the individual, ensuring that goals are actively worked towards and developed, to achieve the maximum level of independence for the individual.

To support the individual to learn to use small pieces of equipment and assistive technology as appropriate to maximise their independence.

To work with individual clients and their families, carers and friends to support them to access their communities.

To work in partnership with other internal and external services and organisations to ensure the optimum independence for the individual.

To undertake and follow relevant risk assessments to ensure work can be carried out in an environment that is safe and conducive to meeting the outcomes for the client.

To undertake ongoing review and monitoring of an individual's progress against the goals set in the Support Plan and report progress.
