Brook Street Social Care alongside a charitable organisation which provides temporary housing and support to individuals and families who have fallen on hard times or are at risk of becoming homeless.


You will be working as a part of a team which provides a customised, mutually agreed support package to vulnerable individuals with a holistic approach in a move on accommodation setting. You will be responsible for supporting a case load of individuals developing personalised support packages with the aim of helping the clients overcome barriers they are currently facing so they can move into and sustain their own housing. You will help the clients overcome the barriers such as lack of skills and confidence that prevent them from living independently.

You will have experience supporting vulnerable adults in care, and ideally hold a minimum NVQ Level 2 in Health and Social Care. You will be resilient, confident with working independently and supportive.

The shifts for this role are on a 6 week rota basis. Day shifts are 8am - 4pm and evening shifts are 3.30pm - 10.30pm. You will not be required to work nights in this role.
