Housing Support Worker

Monday-Sunday 8am-8pm

DBS on update service

Build positive and effective working relationships and agreements with relevant external agencies
- Progress and track referrals from the Local Authority into the service and outcomes for clients moving on from the service.
- Delivery a high-quality off the street accommodation service for individuals rough sleeping within Mendip
- Complete holistic assessments for clients leading to the development of support plans which meet individual client goals.
- Implement person-centred packages of support with clients.
- Give clients advice, information and provide an enabling service so that they can be signposted to and access appropriate services to develop independence
- Provide an effective Housing Management service for clients and identified properties.
