About the Role
As a Support Worker, you will provide person-centred support that promotes independence, dignity, choice, and inclusion. You will build positive relationships with the people you support, helping them achieve their personal goals while ensuring their safety and wellbeing.
Key Responsibilities
- Provide high-quality, person-centred support tailored to individual needs.
- Support individuals with daily living activities, including personal care where required.
- Encourage independence, confidence, and community participation.
- Assist with medication in line with company policies and training.
- Support individuals to attend appointments, education, work, and social activities.
- Promote positive mental health and emotional wellbeing.
- Maintain accurate records and complete daily documentation.
- Work collaboratively with families, healthcare professionals, and other agencies.
- Safeguard vulnerable adults and report any concerns appropriately.
- Follow care plans, risk assessments, and organisational policies.
