Location: Hemel Hempstead
Salary: £30,000 per annum

About the Role
We are seeking a dedicated and organised Care Coordinator to join our CQC-regulated Supported Living Service in Hemel Hempstead. This is an exciting opportunity for an experienced care professional who is passionate about delivering high-quality, person-centred support and ensuring compliance with CQC standards.
As a Care Coordinator, you will play a key role in overseeing care delivery, supporting staff, and ensuring service users receive safe, effective, and compassionate care within supported living settings.
Key Responsibilities
- Coordinate and manage care packages within supported living services
- Develop and review person-centred care plans and risk assessments
- Ensure compliance with CQC regulations and company policies
- Conduct staff supervisions, spot checks, and support visits
- Liaise with families, social workers, healthcare professionals, and external agencies
- Assist with rota management and staffing coordination
- Support with safeguarding concerns and incident reporting
- Maintain accurate and up-to-date care documentation
Requirements
- Previous experience in a Care Coordinator or senior care role
- Experience within Supported Living services
- Strong understanding of CQC regulations and compliance
- Excellent organisational and communication skills
- Ability to manage multiple priorities effectively
- Level 3 or above in Health & Social Care (preferred)
- Full UK driving licence (preferred)
